The experience of care coordination for people living with multimorbidity, and health care professionals: An interpretative phenomenological analysis
This thesis explored the experience of care for people living with multimorbidity, or multiple chronic conditions, who were at risk of hospitalisation. The study incorporated five papers focusing on the experience of care coordination for people living with multimorbidity, in a Hospital Admission Risk Program, and the health care professional experience of providing care coordination. An interpretative phenomenological analytic lens was used to interpret the participants’ perspectives, finding that care focused on a trusting relationship allowed people living with multimorbidity to improve their health and wellbeing. Health care professionals provided care aligned with a framework of relationship-centred care.
