Individualizing cytomegalovirus preventative strategies following solid organ transplantation

In this thesis, we explored predictors of CMV infection in transplant recipients. Lymphopenia was predictive of recurrent CMV, which may be associated with poorer survival. In lung transplant recipients, Quantiferon®-CMV results were correlated with CMV serostatus, and had limited diagnostic utility although we identified a benefit to extended antiviral prophylaxis. Quantiferon®-Monitor was able to identify highly immunosuppressed lung transplant recipients at increased risk for serious opportunistic infections including CMV. These studies support the concept that an individualized approach to CMV prevention and management in transplant recipients is possible and could lead to better outcomes.
