Contribution of medications to hospitalisations from aged care facilities

Residents of aged care facilities are at high risk of hospitalisations due to older age, frailty, multimorbidity and polypharmacy. This thesis identifies medications and prescribing patterns that may contribute to increased hospital admissions in this population. The findings from this thesis will help support the development and implementation of strategies to reduce preventable hospitalisations. The findings will help improve clinical outcomes and quality of life for people living in aged care facilities.
