计划生育家庭 特别扶助
一、 扶助条件：
1、户籍属本辖区，1933年1月1日以后出生;
2、女方年满49岁；
3、只生育一个子女或收养一个子女；
4、现无存活子女或独生子女被依法鉴定为三级以上残疾；
5、因丧偶或离婚的单亲家庭，男方或女方须年满49岁。扶助对象再生育或合法收养子女后，终止扶助。
二、扶助标准：
1、独生子女死亡后未再生育或收养子女的夫妻，由政府给予每人每月不低于500元的扶助，直至亡故为止。
2、独生子女伤、残后未再生育或收养子女的夫妻，由政府给予每人每月不低于400元的扶助金，直至亡故或子女康复为止。
三、办理程序：
1、本人提出申请，填写统一格式的救助审批表；
2、提供夫妻双方身份证、户口本、子女死亡或伤病残证明原件及复印件；
3、居委会和街道办事处进行资格初审后，报区级人口计生行政部门审批并公示。

Special Assistance for Family Planning Families
I. Conditions for support.
1. Household registration belongs to this jurisdiction, born after January 1, 1933;
2. The female party is 49 years of age or older;
3. Only one child is born or one child is adopted.
4. There is no surviving child or the only child has been legally identified as disabled to a level three or above.
5. For single-parent families who are widowed or divorced, the man or woman must be at least 49 years old. The support will be terminated after the recipient has given birth again or legally adopted a child.
II. Criteria for support.
1. Couples who have not given birth again or adopted a child after the death of their only child shall be given a monthly support of not less than RMB 500 per person by the government until their death.
2. Couples who have not given birth to or adopted a child after the injury or disability of their only child shall be given a monthly support of not less than RMB 400 per person by the government until the death or recovery of the child.
3. Application procedures.
1、Application by the person himself/herself, filling in the uniform format of the assistance approval form.
2、Provide the original and photocopies of the couple's identity card, household register and proof of the death or disability of the child.
3、After preliminary examination of eligibility by the neighbourhood committee and street office, the application will be submitted to the district-level administrative department of population and family planning for approval and public announcement
