This thesis represents a series of papers that cover the spectrum of managing survivors of stroke after discharge into the community. Specifically, this research was aimed at determining the benefits of policy and practice options already existing in the Australian health care system for the long-term management of stroke. The major themes of this thesis include managing risk factors after a stroke, improving knowledge of prevention strategies in survivors, and managing unmet needs after a stroke. Evidence from this thesis could help inform policy and practice decisions on how to better support people with stroke in the community.
